If an employee or student is leaving the department, please follow the steps below to ensure a smooth and secure offboarding process:

Update Contact Information: Notify Reginald White at rawhite4@wisc.edu of the individual’s departure so the listserv, Event Management System (EMS), and website can be updated accordingly.

Deactivate Shared Services Access: PIs should inform Shared Services IT to deactivate any accounts associated with the departing individual. Instructions for submitting a ticket are here:

Software Access Removal: Review and remove access to any licensed software platforms. For software management details, refer to:
Keys
Keys must be returned to the office at 420 Henry Mall Rm 1135. Contact Joe Oliva (262-1184, jmoliva@wisc.edu) to confirm the return. The $10 key deposit will be refunded upon return. Failure to return keys may result in a $75 replacement fee.
Building Access
Notify Joe Oliva (262-1184, jmoliva@wisc.edu) to remove building access permissions. You will need to provide the departing individual’s UWID or Wiscard number.
Benefits Termination
The employee will receive information regarding the end of benefit coverage from HR and Payroll. Be aware that most benefit plans end on the last day of employment, with options for COBRA or continuation. For more, visit:

Disability Accommodation
If accommodations were made, ensure all university-owned equipment or furniture provided as part of the accommodation is returned. Contact the BMC Department Administrator or Amy Alston, the School of Medicine and Public Health disability coordinator at 262-1302 for guidance.
